Certified Nurse Aide
1. Performs routine duties on an as-needed basis as required by supervisor in caring for residents.
a. Bathing: Residents are bathed according to the bath schedule-daily for incontinent residents.
b. Dressing: Residents are assisted in dressing, which may require some lifting of residents for putting on pants, shirts, etc. Residents are assisted with putting on and taking off braces as needed.
c. Hair Care: Hair is combed daily and shampooed weekly. For male residents, beards are shaved, trimmed, shampooed and combed daily.
d. Oral Hygiene: Residents are assisted with oral hygiene, including dentures, daily. This may require assisting residents with teeth brushing and may require bending and reaching. Fresh water will be placed in denture cup and sputum cups daily.
e. Nail care: Finger and toenails are trimmed every week and cleaned as needed. For diabetic residents, obtain LVN’s assistance with nail care.
f. Toileting: Residents are assisted as necessary with toileting. Bedside commodes may be kept at the bedside. They are to be emptied and cleaned after each use.
g. Mealtimes: Food trays are served, and residents who need assistance are helped during meal times. Nourishments are served to residents as directed by the charge nurse.
h. Answer every signal light and buzzer promptly and courteously.
2. Observes changes in residents.
a. Any changes in resident skin condition, appetite, behavior or actions will be reported to the charge nurse. Observe all diabetic residents closely. These observations are important to the residents’ care and well being and aid the nurse in evaluating the resident and in formulating his/her care plan, assuring needs are met.
b. Observations to be reported:
1. Signs of illness:
a) Temperature elevation
b) Vomiting or diarrhea
c) Listlessness or lethargy
d) Headache or dizziness
e) Poor color
2. changes in general condition:
a) Pressure areas; open areas
b) Falls, cuts, or abrasions
c) Unusual discharges or bleeding
d) Edema
e) C/O pain or discomfort
f) Changes in bowel or bladder habits
g) Change in physical abilities
h) Change in appetite - difficulty in chewing or swallowing
i) Slurring of speech (sudden onset)
j) Sudden or frequent mood swings
k) Visual or hearing problems
3. Shall keep resident information confidential in accordance with SMRS policies and HIPAA regulations.
